Online Registration

Welcome to Deer Valley Little League Spring Registration. Once you begin the registration process you will proceed through a variety of steps as outlined below:

1. Adopt an Electronic Signature
   Here you will adopt an online electronic signature by entering your name (not your childs), and then choosing a font to represent your siganture. Upon completion of this step you can choose to add an additional signature for your child's other parent/guardian.
2. Registration Form
   The registration form will allow you to enter in details about your child including age, address, school, etc.
3. Medical Release Form
   The medical relase form will allow you to provide us with medical information about your child including primary physician, hospital preference and medications.
4. Register Additional Children
   At this point you will be given the option to fill out the previous two forms for any additional children you are registering (siblings).
5. Code of Conduct
   Read and acknowledge you understand the code of conduct for players, parents and spectators
6. Payment
   You will be taken to a secure site hosted by Authorize.net in order to complete payment and your registration. Upon payment you will receive an e-mail with all the document you have filled out during this process.

Upon Completion of Registration Remember to:

• Drop off birth certificate and 3 proofs of residence at any walk up registration
• All Minors and Majors must sign up for a tryout time
